Living Well Family Medicine is designed to keep pricing simple, honest, and affordable. Below is a list of standard prices insurance charges verses a DPC model. The DPC model removes the barriers (and fears) of cost for visits/services by including services when you become member.

LWFM Membership Pricing:

Age 0-21 $40/mo with accompanying adult membership,

$55/mo w/o adult

Age 22-39 $65/mo

Age 40-64 $80/mo

Age 65+ $95/mo

Family of four $170/mo, ($25.00/mo for each additional child)

There is a one-time start-up fee of $70 per patient ($200 max per family) in order to gather, review, and update all medical records.

Home visits are available if deemed appropriate by the physician and based on physician availability. An extra charge will apply.

PLEASE NOTE: We offer a $5.00 dollar per month discount for patients that choose to use direct bank transfer instead of a credit card or HSA card.
